The patient had various medications in different doses, but there was an error with two different medications that spanned all the days of the week in the dosette box. The mistake was reported to the Health and Social Care Inspectorate, which now criticises the district nurse who made the error. She herself has explained that it happened because she was interrupted while organising the medications in the dosette box.
