Section 867...The insurance policy must be signed by the insurer and contain the following items: (1) the subject of the insurance; (2) the risk taken by the insurer; (3) the value of the insurable interest, if fixed; (4) the sum insured; (5) the amount of the premium and the methods of its payment; (6) the commencing time and terminating time of the insurance contract, if it is governed by a time limit; (7) the name or trade name of the insurer; (8) the name or trade name of the insured; (9) the name of the beneficiary, if any; (10) the date of conclusion of the insurance contract; (11) the place and date of making of the insurance policy.
